After you submit the form you should see a message in a box with a  green border saying “Your registration was submitted successfully. Thanks.”, and you will be re-directed to a “thank you” page. Once you have submitted the form successfully, you will also receive a copy of the submitted form by email for your records.

If you instead receive this message: “One or more fields have an error”, please check you’ve completed all mandatory fields marked with * and then resubmit the form.
